高度近视全阈值静态中心视野改变的研究

摘    要:为研究高度近视眼全阈值静态中心视野的改变,应用DICONTKS-4000型自动视野分析仪检查48例(90眼)的全阈值静态中心视野并观察其后极部的眼底改变,同时以低中度近视眼作为对照。结果:高度近视眼的中心视野损害与患者的屈光度、年龄及后极部眼底改变有关。检查发现78眼(86.7%)视野异常,其中59眼(65.6%)生理盲点扩大,黄斑中心相对暗点53眼(58.9%),弥漫性损害28眼(31.1%)。视野损害以颞侧明显。视野改变的形态、程度与眼底后极部改变的形态不完全对应。高度近视眼黄斑中心相对暗点出现早于眼底镜下黄斑部病变。结论:高度近视患者视野改变早于眼底镜下眼底改变,对于早期诊断及治疗具有指导意义。

Abstract:Objective:To study the changes of Automated static central visual defects in high myopia.Methods:The static central visual field tested by DICON TKS 4000 Field Analyzer and posterior polar lesions in 48 cases(90eyes)with hight myopia were investigated.Cases with low and moderate degrees of myopia were the controls.Results:The visual field defects are present in the high myopia eyes and they are related to the degree of high myopia ,the age of the patient and the severity of the postorior polar lesion.Among 90 eyes of high myopia,78 eyes(86 7%)showed field abnormalities.Enlargement of the blind spot occurred in 59 eyes(65 6%),central relative scotomata 53 eyes(58 9%)and diffuse field defects 28 eyes(31 1%).The temporal bemifield,particulary the upper temporal quadrant appeared to be most affected.The appearance of central relative scotomata in high myopia eyes detected by a quantified automated permeter is prior to the appearance of macular lesions seen under an ophthalmoscope.Conclusion.The quantified automated central visual field examination is helpful to the early diagnosis of macular disease in high myopia.
