术后诱发外科危象急重症的治疗走向

目的对甲亢、胸腺瘤、肾上腺肿瘤的术后并发外科危象的病因和诱因分析及围术期处理的探讨。方法对24例术后外科危象临床资料进行回顾性分析与总结。结果甲亢行双侧甲状腺大部切除术后并发外科危象10例,9例治愈,1例死亡;重症肌无力合并胸腺瘤术后并发外科危象8例,死亡1例,7例治愈;肾上腺皮质良恶性肿瘤术后并发外科危象6例,均获痊愈。结论完善围术期处理,减少外科危象的发生,适时综合治疗和正确给药时间、用量方面的选择,是提高外科危象的抢救成功率,降低死亡率的好方法。

Therapeutic tendency of severe acute surgical crisis after the operation

Objective To analyse the reasons of postoperative surgical crisis of endocrinopathy and to discuss the preparation before surgical operation and the treatment after surgical operation. Methods To analyse and summarize retrospectively clinical data of 24 cases with surgical crisis. Results 10 cases of postoperative surgical crisis of big partial thyroidectomy, 9 cases were cured and 1 case died;8 cases of postoperative surgical crisis of myasthenia and excision of thymoma, 7 cases were cured and 1 died;all 6 cases of surgical crisis of benign and malignant tumors of adrenal glands had recovered. Conclusion To improve the perioperative treatment, to reduce surgical crisis, to treat timely and to choose correct time and dose will improve the success rate of surgical crisis and reduce the mortality .
