特发性血小板减少性紫癜患者预防性血小板输注阈值的探讨

目的：探讨特发性血小板减少性紫癜（ITP）患者预防性血小板输注的阈值。方法：将入院时出血程度为0级的ITP患者分为A、B两组，A组：血小板〈10×10^9/L，B组：血小板（10～20）×10^9/L，比较两组输注血小板的次数和量、出血情况、输血小板的不良反应。结果：A组输血小板次数和量低于B组，A、B两组出血事件发生率无显著性差异。A组输血小板不良反应发生率低于B组。结论：ITP患者在无侵袭性操作的情况下，采用血小板数10×10^9/L作为预防性血小板输注的阈值是安全的，且可减少输注血小板的输注量和不良反应。

Evaluation of Threshold for Prophylactic Platelet Transfusions in Patients with ITP

Objective：To evaluate the threshold for prophylactic platelet transfusions in patients with ITP. Methods： ITP patients admitted with bleeding severity of grade 0 were divided into Group A and Group B. Group A： platelet count 〈 10×10^9/L, Group B： platelet count （10-20）×10^9/L. The times and numbers of platelet transfusions, bleeding events, adverse reactions of platelet transfusions were compared between the two groups. Results：The times and numbers of platelet transfusions in Group A were fewer than those in Group B. Bleeding events were not significantly different between the two groups. The rates of adverse reactions of platelet transfusions in Group A were lower than those in Group B. Conclusion：On condition that the invasive procedures were not needed in patients with ITP, it was safe to set the threshold for prophylactic platelet transfusions at platelet count of 10 ×10^9/L,and it can reduce the numbers and adverse reactions of platelet transfusions.
