氟比洛芬酯用于妇科腹腔镜手术的超前镇痛观察

目的：观察氟比洛芬酯对于妇科腹腔镜手术患者的超前镇痛作用。方法：120例ASAⅠ级择期行妇科腹腔镜手术患者随机分为A、B、C3组，每组各40例，均采取气管内插管全麻。A组在术前30min静注1mg／kg的氟比洛芬酯，手术后静注生理盐水10ml；B组在手术前30min静注生理盐水10ml，手术后静注1mg／kg的氟比洛芬酯；C组在手术前后均静注生理盐水10ml。术后用视觉模拟评分法（VAS）单盲评估患者术后0．5h、1h、2h、6h、12h、24h和48h的疼痛感觉程度，并观察与氟比洛芬酯有关的不良反应。结果：A组术后24hVAS评分显著低于C组沪〈0．05），B组0．5h、1h、2hVAS评分与C组无显著性差异（P〉0．05），6h、12h、24h显著低于C组（均P〈0．05），3组48hVAS评分无显著性差异（P〉0．05）。结论：氟比洛芬酯术前静脉应用有良好的超前镇痛作用，不良反应少见，对于创伤小的手术术后镇痛和多模式互补镇痛有一定的应用价值．但单一的术前静脉给药并不能减轻或抑制患者整个围术期疼痛．

Flurbiprofen used for preemptive analgesia in patients undergo gynecological laparoscopic surgery

Objective:To assess the effects of preemptive analgesia with flurbiprofen on the postoperative pain for patients who were undergoing gynecological laparoscopic surgery.Methods:One hundred and twenty cases scheduled for selective gynecological laparoscopic surgery were randomly divided into A,B and C groups.Thirty minutes before operation,the patients in group A received flurbiprofen 1mg/kg intravenously and group B,group C received normal saline 10ml.After operation,the patients in group B received flurbiprofen 1mg/kg and group A,group C received normal saline 10ml.The effects of analgesia after operation were assessed by visual analogue scale(VAS)at 0.5,1,2,6,12,24,48 hours after operation,and side effects were also assessed.Results:The VAS of group A was lower than that of group C during 24 hours after operation(P<0.05),The VAS were same between group B and group C at the time point of 0.5,1,2 hour(P>0.05).The VAS of group B was lower than that of group C at the time point of 6,12,24 hour(P<0.05).The VAS were same in the three groups at the time point of 24 hour(P>0.05).Conclusion:Preemptive analgesia with flurbiprofen 1mg/kg could provide satisfactory pain relief with no obvious side effects during 24 hours after gynecological laparoscopic surgery.We should continue to use flurbiprofen in order to relieve postoperative pain and establish multimode complementation analgesia in laparoscopic surgery.But we can not relieve or inhibit the pain comletely by simple preoperative use flurbiprofen.
