高分辨率CT扫描对外鼻骨折的诊断

目的：复习分析高分辨率CT（HRCT）扫描及辅以三维重建（3D成像）对外鼻骨折的诊断,并参照常规CT平扫、X线片,论述其诊断价值。方法：对50例鼻外伤患者的鼻骨X线平片、常规CT扫描及HRCT扫描（其中15例行3D成像）图像分析,双盲法阅片,研讨HRCT扫描及辅以3D成像对外鼻骨折的诊断优势。结果：以HRCT扫描辅以3D成像综合分析作为外鼻骨折诊断金标准。对外鼻骨折的显示率,X线平片,常规CT平扫、HRCT扫描分别是：69.3%（34/49）、85.7%(42/49),95.9%(47/49)。其中X线平片对外鼻骨折的具体位置,骨折程度分析能力最低,常规CT平扫其次,HRCT扫描相对准确率最高。结论：HRCT扫描对外鼻骨折的显示具有相对优势,如必要时辅以3D成像则可作诊断的金标准。

Diagnoses in the External Nose Fracture by High-resolution CT Scanning Method

Objective:The high resolution CT(HRCT) with three dimension reconstruction(3D) in the external nose fracture diagnoses were reviewed and analyzed,and its diagnosis advantages were also discussed compared to the conventional CT scanning and X ray image.Methods:50 patients with the nose external injury were examined by the X ray,conventional CT scanning and HRCT scanning.(Among 50 patients,15 patients were examined with 3D reconstruction).The double blind reading image methods were used.The diagnosis advantages of HRCT scanning with 3D reconstruction were discussed.Results:The HRCT scanning accompanying with 3D image analyses is the best way to diagnose the external nose bone fracture.The accurate rates of external nose fracture diagnoses are:69.3%(34/49)for X ray image,85.7%(42/49) for conventional CT scanning,95.9%(47/49) for HRCT scanning,respectively.It is clear that the diagnosies rate of X ray image is the lowest to judge the position and degree of external nose fracture,and the conventional CT scanning is better,and the HRCT scanning is the best method.Conclusion:HRCT scanning has a distinct advantage to diagnose the external nose fracture,and if necessary,HRCT scanning together with 3D image is the "golden standard"to diagnose the nose fracture.
