急性冠状动脉综合征患者血浆中溶血磷脂酸水平的临床研究

目的　探讨急性冠状动脉综合征 (ACS)患者血浆中溶血磷脂酸 (LPA)水平的变化。方法　入选 12 0例ACS患者 ,根据病情将其分为急性心肌梗死 (AMI)组 6 0例 ,不稳定性心绞痛 (UAP)组 6 0例 (据抗凝及抗血小板治疗前UAP为UAP1 组 ,治疗后的UAP为UAP2 组 ) ,并入选 6 0例健康者作为对照组 ,分别测定血浆中LPA。结果　AMI组和UAP1 组血浆LPA水平分别是 (3.80 17± 0 .6 2 89) μmol L、(3.0 2 73± 0 .76 2 3) μmol L],均显著高于对照组 (1.8378±0 .6 75 3) μmol L],P <0 .0 5 ,其中异常增高率AMI组占 6 3.3%,UAP1 组占 45 %,UAP2 组占 13.3%,正常组 5 %,UAP抗凝抗血小板治疗后LPA明显降低 (2 .0 0 48± 0 .8470 ) μmol L。结论　血浆中LPA作为体内凝血和血栓形成过程早期释放的分子标记物在预测ACS的发生发展及指导合理应用抗血小板及抗凝药物方面可能有一定应用前景。

The Clinical study of plasma lysophosphatidic acid in patients with acute coronary syndromes

Objective To study the level of plasma lysophosphatidic acid(LPA) in patients with acute coronary syndromes(ACS).Methods 120 patients with ACS were devided into two groups:group AMI(AMI) 60 and group unstable angina(UAP) 60(before treatment UAP 1 and after treatment UAP 2),and 60 healthy persons served as control.LPA was extracted from plasma of patients.Results Plasma LPA levels of AMI and UAP 1 were 3.8017±0.6289 μmol/L and 3.0273±0.7623 μmol/L respectively,significantly higher than that in control group(1.8378±0.6753 μmol/L),P<0.05.The plasma LPA level of UAP decreased significantly after anticoagulant and antiplatelet treatment(2.0048±0.8470 μmol/L).Conclusion As a molecular marker released by activated platelet in vivo,plasma LPA may be valuable for predicting the occurrence and development of ACS and guiding the treatment.
