不阻断腹主动脉和下腔静脉的 Fish-Lewis大鼠原位肾移植慢性排斥模型的建立

摘    要:目的] 建立大鼠原位肾移植研究肾移植慢性排斥的模型. 方法] 自 1998年 8月至 2003年 1月,以 Fisher大鼠作为供体, Lewis大鼠为受体.模拟活体供肾肾移植中供受体同步手术的顺序,采用显微外科技术,进行不阻断腹主动脉和下腔静脉的大鼠原位肾移植,共完成 650例. 结果] 此模型并发症少,移植肾术后成活率达 98.2%.术后第 14天; 1个月; 2个月; 3个月; 4个月; 5个月和 6个月成活率分别为 96.8%; 95.8%; 95.4%; 95.4%; 94%; 94%和 92.5%.术后 10~ 12周,开始出现蛋白尿,血肌酐升高的典型慢性排斥反应. 结论] 此模型具有手术简单、成功率高、影响肾功能因素少、结果稳定等优点.适应于研究肾移植慢性排斥反应.

Abstract:ObjectiveTo set up a renal orthotopic transplantation model in rat for studying renal transplantation chronic rejection.MethodsKidney transplantation model was established using Fisher rats as donors and Lewis rats as recipients. Under the microscope, the procedure was similar with living donor renal transplantation in human, but not heterotopic transplantation. During the whole procedure, the aorta and inferior vena cava were not be interdicted. After operation, from day 0 to day 9, low dose of cyclosporine A (CsA 1.5 mg/kg) was injected subcutaneously for preventing acute rejection reaction; at day 10, the right kidneys in recipients were removed. ResultsFrom Aug. 1998 to Jan. 2003, the authors finished 560 kidney transplantations. The operating time was 45-60 minutes. The survival rates of allograft were 98.2%. After operation, at day 14, one month, two months, three months, four months, five months, and six months, the survival rates were 96.8%, 95.8%, 95.4%, 95.4%, 94%, 94%, and 92.5%, respectively. After operation, at 10-12 week, there were proteinuria and high blood creatinine in recipitients. The kidneys transplanted were showed typical chronic rejection. ConclusionsThis model have many advantages: easy to learn, high survival rates and nearly not to influence renal function. It is a useful model in studying chronic rejection of renal transplantation.
