胱抑素C与冠状动脉慢血流现象的相关性

摘    要:目的探讨胱抑素C与冠状动脉慢血流(CSF)现象的相关性。方法选择冠状动脉造影检查诊断为CSF的患者34例(CSF组)和冠状动脉造影血管正常且血流正常患者35例(对照组),采用校正的心肌梗死溶栓治疗(TIMI)血流记帧法判定冠状动脉血流速度,分别测定两组患者的血超敏C反应蛋白、尿酸和胱抑素C等。结果两组性别构成、年龄、吸烟史、高血压、糖尿病、冠心病家族史、低密度脂蛋白胆固醇、d脂蛋白比较差异无统计学意义(P〉0.05)。CSF组超敏C反应蛋白、胱抑素C和尿酸明显高于对照组[(4.85±6.39)mg/L比(2.55±2.18)mg/L、(0.87±0.22)mg/L比(0.75±0.16)mg/L、(329.68±85.46)μmol/L比(278.97±76.74)μmo//L],差异有统计学意义(P〈0.05或〈0.01)。二元Logistic回归分析结果显示,胱抑素C升高是CSF的独立危险因素(P=0.002,OR=0.009)。结论胱抑素C升高可能参与了CSF的发生、发展过程。

Abstract:Objective To explore the correlation between serum level of cystatin C and coronary slow flow (CSF). Methods Thirty-four patients with CSF were enrolled in CSF group and thirty-five patients with normal coronary flow and angiographically normal coronary arteries were enrolled in control group. Coronary flow patterns was assessed by corrected thrombolysis in myocardial infarction (TIMI) frame count. The change of serum high sensitivity C-reactive protein, uric acid, cystatin C were measured. Results There was no significant difference between two groups with respect to gender, age, history of smoking, prevalence of hypertension and diabetes mellitus,family history of coronary heart disease,low density lipoprotein, ct-lipoprotein (P 〉 0.05). Compared with control group,the level of serum high sensitivity C-reactive protein, eystatin C, uric acid in CSF group were obviously higher [(4.85 ± 6.39) mg/L vs. (2.55 ± 2.18) mg/L, (0.87±0.22) mg/L vs.(0.75 ± 0.16) mg/L, (329.68 ±85.46) μmol/L vs. (278.97± 76.74) mo//L] (P 〈 0.05 or 〈 0.01 ). Logistic regression analysis showed that cystatin C increased as independent risk factors for CSF (P = 0.002, OR = 0.009). Conclusion High level of eystatin C may play an important role in the occurrence and development of CSF.
