脑室出血的手术治疗

目的介绍脑室出血患者的手术治疗经验。方法对吉林省脑科医院（1996—2008年）收治的57例脑室出血病人进行回顾性分析。结果脑室出血病人起病急、进展快，把握好手术适应症，运用脑室外引流手术，术中采用尿激酶注入脑室，术后脑室外引流配合腰穿放液，完全恢复39例（68％），生活自理8例（14％），术中、术后再出血死亡7例（12％），感染3例（5％）其中2例感染病例死亡（3％）。结论脑室外引流是治疗脑室出血的有效办法，对出血已稳定的病例术中向脑室内注尿激酶以利凝血块溶化引流，术后脑室外引流配合腰穿放出血性脑脊液。

The surgical treatment for intraventricular hemorrhage

Objective To introduce the experience of the surgery in the intraventricular hemorrhage. Methods To make a retrospective study of 57patients of intraventricular hemorrhage in the Cerebral Hospital of Jilin province（1996- 2008 years followed-up）. Results Fully recovered in 39 cases（68%）, 8 cases of self-care （14%）, 7 patients died which caused by intra-operative or post-operative rebleeding（12%）, 3 cases of infection（5%）（of which two cases died）. Conclusion The external ventricular drainage（EVD） is an effective mean to treat intraventricular hemorrhage, to inject urohinase during operation for the benefit of melting blocking blood as the cases who have been in the stable condition. Lumbar vertebrae puncture with cerebrospinal fluid and external ventricular drainage during post-operative care are also necessary.
