胆囊及胆管结石CT与MRCP诊断价值分析

摘    要:目的 评价CT与MRCP在胆囊结石及胆管结石中的应用价值。 方法 选择2003年1月至2004年2月期间怀疑胆系结石行MRCP检查的病人中,同时具有CT检查资料且最终确诊的患者共66例作为研究对象,年龄范围27-80岁,平均年龄46.3岁,将MRCP与CT对胆囊结石及胆管结石的显示率分别进行统计学分析。 结果 在胆囊结石的显示上,CT与MRCP的显示率分别为92.45%、86.79%,两者的差异无显著性意义(P>0.05);而对于胆管结石,显示率分别为33.33%、87.50%,两者的差异有显著性意义(P<0.001)。结论 CT与MRCP在显示胆囊结石上,两者具有同样较高的诊断价值,而在显示胆总管结石上,MRCP的临床应用价值远高于CT。

Abstract:Objective To evaluate the application of CT and MRCP in demonstration of cholecystolilhiasis and choledocholithiasis. Methods sixty-six patients (ranging from 27-80 years old; mean age 46.3 years) with cholecystolithiasis and choledocholithiasis or both were studied with CT and MRCP. The sensitivity of CT and MRCP was calculated and compared. Results Cholecystic stones were revealed with CT and MRCP images in 49(92.42%) and 46 patients (86.79%) (P >0.05) respectively. The difference of sensitivities between the two methods was not significant. In choledocholithiasis, the sensitivity was 33.33% and 87.50% (P <0.001) for CT and MRCP respectively. The sensitivity of MRCP was significantly higher than that of CT. Conclusion Our results indicate that CT and MRCP have similar value in detecting cholecystolithiasis. But for the detection of choledocholithiasis, MRCP are significantly more sensitive than CT.
