从痰瘀论治消渴初探

糖尿病属中医"消渴"范畴.其病因病机偏重于燥热偏盛,阴津亏耗;一般认为以阴盛为本,燥热为标,两者往往互为因果,燥热越盛则阴津越虚,阴津虚则燥热越盛.病变的脏腑主要在于肺、胃、肾.临床可见口渴喜饮、小便频数、浑浊而甜,消谷善饥、大便干结、疲乏消瘦.消渴病迁延日久,气阴两伤,痰浊瘀血痹阻脉络,可导致其他脏腑的多种病变.若目络瘀阻,血不循经或精不上承于目,则视物模糊、眼底出血,甚至目盲失明;若痰浊瘀血痹阻四肢脉络,则肢体麻痛,甚至肢端坏疽;若皮肤失养,脉络郁阻,兼感受风湿毒邪,则可出现皮肤瘙痒、溃疡等多种病变.治法为化痰活血,逐瘀通络,方用黄连温胆汤加减.药用黄连、竹茹、胆南星、枳实等.
