肾结核50例分析

目的探讨临床肾结核的诊断和治疗。方法回顾性分析肾结核病例50例临床资料。结果膀胱刺激症状、血尿、腰痛是最常见临床症状。本组采用药物治疗23例,均采用异烟井 利福平 吡嗪酰胺,三联治疗疗程6～9个月。27例手术治疗行肾、输尿管切除。术后抗结核治疗半年以上。结论对于不典型肾结核诊断应尽早采取联合实验室检查方法,使其诊断率高。IVU是肾结核诊断的首选影像学检查,CT对诊断早期肾实质结核病灶有重要价值,IVU不显影时,CT更具有优越性。早期肾结核短程三联药物治疗效果满意,肾结核肾切除应尽可能切除患处输尿管。

A clinical study of 50 cases of renal tuberculosis

[Objective] To study the diagnosis and treatment of renal tuberculosis. [Methods] A retrospective study was made on 50 cases. [Results] Irritation symptoms, hematuria and lumbodynia were the common symptoms. 23 of the 50 cases were cured by medicine (INH RFP PZA for 6-9 months). Nephro-ureterectomy was performed for 27 cases. [Conclusions] Combined use of different means of diagnosis technique would help to assess the diagnosis of renal TB especially in atypical cases. IVU is still the first choice for diagnosis and CT scan is helpful for early cases. INH, REP and PZA used combinedly yiels satisfactory outcome for early cases. When nephro-ureterectomy is indicated, the involved ureter shoul be excised as much as possible.
