肿瘤标志物联合检测对肺癌早期诊断及病理分型的意义

摘    要:目的探讨肿瘤标志物联合检测对肺癌早期诊断的意义及与肺癌不同病理组织类型的关系。方法选择2010年9月—2011年11月肺癌患者91例为肺癌组,同期住院的45例肺部良性疾病患者为肺部良性疾病组,36名健康人为对照组,采用电化学发光法对其血清进行血清肿瘤标志物神经元特异性烯醇化酶(neuron-specific enolase,NSE)、细胞角蛋白(cytokeratin 19 fragment,CYFRA21-1)、癌胚抗原(carcinoembryonic antigen,CEA)、糖类抗原125(carbohydrate antigen,CA125)联合检测,比较3组检测结果及单项与联合检测的灵敏度、特异性和准确性。结果肺癌组4种肿瘤标志物水平均明显高于肺部良性疾病组和对照组(均P<0.05),4种肿瘤标志物联合检测对肺癌诊断的灵敏度、特异性和准确性分别为91.2%、81.3%和88.4%,灵敏度和准确性明显大于各标志物单项检测。结论 NSE、CYFRA21-1、CEA、CA125联合检测可提高肺癌的早期诊断率和准确率,对鉴别肺癌的病理类型有一定价值。

Abstract:Objective To investigate the relationship of combined detection of tumor markers for early diagnosis of lung cancer and pathologic classification.Methods Exampled 91 lung cancer patients from September 2010 to November 2011 as the cancer group,45 patients lungs healthy as the benign disease group,36 healthy people as the comparison group.Then using chemiluminescence method to detect the serum tumor markers in NSE,CYFRA21-1,CEA and CA125,compare and contrast the results.Results Four kinds of tumor markers levels of the lung cancer patients were significantly higher than those of healthy controls and benign lung diseases(all P〈0.05),Sensitivity,specificity and accuracy of combined detection for lung cancer diagnosis were 91.2%,81.3% and 88.4%.Sensitivity and accuracy were significantly higher than that of the marker single detection.Conclusion NSE、CYFRA21-1、CEA and CA125 combined detection can enchance the early diagnosis rate and accurate rate of lung cancer and has a certain value in identification of the lung cancer pathological type.
