摘    要:目的探讨肾上腺血肿的多层螺旋CT(MSCT)征象及其诊断价值。方法回顾性分析经手术或CT随访证实的32例患者的CT资料,采用MSCT机平扫及增强扫描。结果 32例患者中,单纯右侧肾上腺血肿21例(65.6%),单纯左侧8例(25.0%),双侧3例(9.4%);外伤23例(71.9%),非外伤9例(28.1%);肾上腺圆形、类圆形高密度血肿24例(75.0%),肾上腺弥漫性出血5例(15.6%),肾上腺肿胀密度增高3例(9.4%)。动态增强三期扫描均未强化。结论多层螺旋CT能准确显示肾上腺血肿的大小、形态及密度,对肾上腺血肿的早期诊断及鉴别诊断具有重要意义。

Abstract:Objective To discuss the feature and diagnostic value of multislice spiral CT(MSCT) in adrenal hematoma.Methods A retrospective analysis was carried out in 32 cases with adrenal hematoma confirmed by surgery or follow-up CT,whose CT data were obtained from MSCT plain and enhanced scan.Results Out of 32 cases with adrenal hematoma,21 were found to have right adrenal hematoma(65.6%),8 left adrenal hematoma(25.0%),and 3 bilateral adrenal hematoma(9.4%);23 cases were traumatic(71.9%) and 9 non-traumatic(28.1%).Adrenal hematomas appeared as round or oval hyperdense mass in 24 cases(75.0%).Diffuse irregular hemorrhage occurred in 5 cases(15.6%),swollen gland with higher density in 3 cases(9.4%).All images were not enhanced with three-phase contrast-enhanced dynamic scan.Conclusions MSCT scan can accurately demonstrate the size,shape and density of adrenal hematoma and is of great significance to its early diagnosis and differential diagnosis.
