中西医结合治疗对急性缺血性脑卒中的临床综合干预效果评价

目的:观察中西医结合治疗急性缺血性脑卒中的临床疗效及血清超敏C反应蛋白(hs-CRP)、血液流变学指标的变化,客观评价中西医结合治疗对急性缺血性脑卒中的整体效果。方法:180例缺血性脑卒中患者随机分为中西医结合组(实验组)和西医组(对照组),每组各90例,实验组在西医常规治疗基础上加入中药、针灸等疗法;对照组西医常规治疗加早期康复锻炼。两组均为14d一疗程。取两个治疗组治疗前、治疗后7d以及治疗后14d三个时间段进行比较,分别测定两组患者的美国国立卫生研究院卒中量表(NIHSS)评分、改良的Rankin量表(MRS)评分、血液流变学及血清hs-CRP指标的变化。结果:(1)实验组治疗后7d和14d与对照组相比,实验组的卒中量表评分和改良的Rankin量表评分改善较明显,差异有统计学意义(P<0.01)。(2)两组患者治疗后血清hs-CRP水平、血液流变学各项指标均较治疗前明显改善,实验组改善程度好于对照组,差异均有统计学意义(P<0.01)。结论:中西医结合治疗可改善急性缺血性脑卒中神经功能缺损,降低急性缺血性脑卒中血清hs-CRP水平,改善血液动力学状态,改善患者预后。

Effect of combine traditional Chinese and western medicine treatment on acute ischemic stroke

Objective: To observe the effect of combination of TCM and western medicine on acute ischemic stroke and on serum super sensitive c-reactive protein(hs-CRP),blood rheological indexes.Methods:A total of 180 cases with ischemic stroke were randomly divided into combination treatment(experimental group) and western medicine group(control group),with 90 cases in each group.Patients in experimental group had TCM and acupuncture therapy on the basis of conventional western medicine treatment;patients in control group had conventional western medicine treatment and early rehabilitation exercise.All patients had treatment for 14 d.The course included three sections: 7 d before and after treatment,and 14 d after treatment.United States national institutes of health stroke scale(NIHSS) scores score,modified Rankin scale(MRS) score,blood rheological indexes and serum hs-CRP level were measured and compared.Results:NIHSS score and modified Rankin score was significantly improved after treatment in the experimental group,and the difference was significant as compared with the control group(P<0.01).Blood rheological indexes and serum hs-CRP level was significantly improved after treatment in two groups,and the improvement was more significant in experimental group(P<0.01).Conclusion:The combination of TCM and western medicine therapy can improve acute ischaemic stroke nerve function defect,reduce acute ischaemic stroke serum hs-CRP level,improve blood dynamic state,and improve the prognosis of patients.
