无痛人流术对患者Th1/Th2细胞因子平衡的影响

目的:观察无痛人流术患者血浆Th1/Th2细胞因子的变化,评价麻醉下人工流产术对患者应激反应及免疫功能的影响。方法:选择做人工流产手术患者60例,随机分为2组,无痛人流组(A组)和普通人流组(B组),每组各30例,分别于开始手术前(T1)、扩宫口时(T2)、手术结束后30min(T3)三个时点,在患者非输液侧前臂采集静脉血2mL,采用ELASA法测定血浆IFN-γ和IL-10浓度。结果:丙泊酚-芬太尼静脉麻醉用于人工流产手术患者IFN-γ和IL-10浓度变化不大,而无麻醉组IFN-γ浓度显著降低,IL-10浓度显著升高。结论:无痛人流术减少了患者的痛苦,术中便于操作,缩短了手术时间,预防人工流产综合征的发生,降低了病人因手术刺激引起的血浆IFN-γ和IL-10变化,有效抑制应激反应,保护机体免疫功能。

Influence of painless abortion surgery on Th1/Th2 cytokine balance

Objective:To observe the influence of painless abortion surgery on Th1/Th2 cytokine balance and to evaluate the influence of artificial abortion under anesthesia on stress response and immune function.Methods:Sixty patients scheduled for artificial abortion were randomly divided into painless abortion group(group A) and routine abortion group(B),with 30 cases in each group.At three time points including:before the surgery(T1),time for expansion of cervix((T2) and 30 min after the end of surgery(T3),collect 2 mL non-transfusion blood from the forearm of patients for IFN-γ and IL-10 concentration determination by ELASA method.Results:Group A which underwent surgery under anesthesia with fentanyl and propofol showed little change in IFN-γ and IL-10 concentration,but in group B,IFN-γ concentration significantly decreased and IL-10 concentration significantly increased.Conclusion:Painless abortion is less time consuming but more comfortable than routine abortion surgery,it induce less complications and has less influence on IFN-γ and IL-10 concentrations,can effectively inhibit stress response to protect the body's immune function.
