功能锻炼图谱在老年心衰患者下肢水肿中的应用效果研究

目的探讨下肢功能锻炼图谱在老年心衰患者下肢水肿中的应用效果。方法选取2016年7月—2017年10月收治于复旦大学附属中山医院心衰伴下肢水肿患者61例,随机分为观察组(n=31)和对照组(n=30)。对照组给予常规心衰治疗健康宣教,进行日常活动;观察组在此基础上,根据下肢功能锻炼图谱,指导患者进行功能锻炼。比较两组患者锻炼1周、2周后踝关节、踝上10 cm的小腿周径;比较两组患者干预前后明尼苏达心衰生活质量量表(MLHFQ)评分。结果干预1周后,观察组右侧脚踝及小腿周径显著小于对照组(P0.05);干预2周后,观察组双侧脚踝及小腿周径均显著小于对照组(P0.05)。干预后,观察组MLHFQ量表评分显著优于对照组,差异有统计学意义(P 0.01)。结论下肢功能锻炼图谱有利于改善老年心衰患者下肢水肿,提高患者生存质量。

Objective To evaluate the effects of lower limb functional exercise graph in alleviating lower extremity edema in elder heart failure patients. Methods: 61 elder patients with heart failure hospitalized in geriatrics department from July 2016 to October 2017 were randomly assigned to the control group (n=30) and the study group (n=31). The control group underwent routine education and activity. The study group was subjected to functional exercise according to lower limb functional exercise graph. Circumferences at ankle and 10cm above ankle were recorded at baseline, 1 week and 2 weeks after treatment for both groups. Minnesota living with heart failure questionnaire (MLHFQ) was also compared between these two groups. Results: One week after intervention, the circumferences at right ankle and 10cm above right ankle were significantly shortened in study group than in control group (P<0.05). 2 weeks after intervention, circumferences at both ankle and 10cm above both ankle were significantly shortened in study group than in control group (P<0.05). Patients in study group also showed significantly higher MLHFQ scores compared with control group (58.50±0.51 vs 44.58±0.67, p<0.001). Conclusion: Functional exercise graph helps to alleviate lower limb edema in elder heart failure patients and to improve their life quality.
