多层螺旋CT在骨关节创伤中的应用价值

摘    要:目的 探讨多层螺旋CT在骨关节创伤中的诊断价值。方法 78例骨关节创伤患者,全部行多层螺旋CT薄层横断面扫描,部分行三维重建(包括VR法和SSD法)及多平面重建(MPR),分析各方法对骨折或脱位的显示情况。结果 肩、肘、膝、踝关节及骨盆创伤78例,CT横断面扫描均获明确诊断;所有病例均行SSD重建,明确诊断75例;71例行VR重建,明确诊断70例;37例使用了MPR重建,均获明确诊断。图像显示多层螺旋CT横断面扫描可发现细小骨折但缺乏立体信息,3D重建立体感强,可多角度多方向观察,清楚直观;MPR法显示骨折线走行和移位更有优势且能兼顾软组织改变。结论 多层螺旋CT多种重建方法各具特色,结合使用可提高骨关节创伤的诊断准确性,简化思维,为临床诊治提供直观的立体信息。

Abstract:Objective To evaluate diagnostic value of multislice spiral CT (MSCT) on trauma of bones and joints.Methods Seventy eight patients with fracture or dislocation were underwent thin slice volume scan by MSCT,then performed reconstruction in ways of multi planer reconstruction (MPR),surface shaded display (SSD),volume rendering (VR),and the results of different methods of reconstruction were analyzed.Results All of the 78 patients got correct diagnosis by axial CT scan; SSD diagnosed 75 cases; 71 cases were performed VR and 70 of them were diagnosed accurately; 37 cases performed MPR were all diagnosed accurately.Axial images of MSCT could show tiny fracture,3D images had more advantages in displaying space relationships of the fracture and dislocation.MPR was superior in displaying courses of fracture line without losing the information of soft tissue.Conclusion MSCT is a very useful technique in diagnosis of trauma of bones and joints.Combination of all techniques of reconstruction can increase the diagnostic accuracy,3D images can also simplify the course of diagnosis and can give more help to the clinical treatment.
