肺透明膜病患儿发生支气管肺发育不良的影响因素分析

目的探讨肺透明膜病(HMD)患儿发生支气管肺发育不良(BPD)的影响因素。方法回顾性分析2018年1月至2020年12月我院收治的451例HMD患儿的临床资料,根据BPD发生情况分为BPD组与非BPD组,分析HMD患儿发生BPD的影响因素。结果451例HMD患儿中80例发生BPD,占比为17.74%。Logistic回归分析显示,胎龄≤32周、出生体重≤1.5kg、合并宫内感染、肺出血、机械通气时间>6 d是HMD患儿发生BPD的独立危险因素(P<0.05,OR>1)。结论胎龄≤32周、出生体重≤1.5 kg、合并宫内感染、肺出血、机械通气时间>6 d是HMD患儿发生BPD的独立危险因素,临床应针对危险因素加强治疗与预防,以降低BPD发生率。

Analysis on the Influencing Factors for the Occurrence of Bronchopulmonary Dysplasia in Children with Hyaline Membrane Disease

Objective To explore the influencing factors for the occurrence of bronchopulmonary dysplasia(BPD)in children with hyaline membrane disease(HMD).Methods The clinical data of 451 children with HMD admitted to our hospital from January 2018 to December 2020 were retrospectively analyzed.According to the occurrence of BPD,all cases were divided into BPD group and non-BPD group.The influencing factors for the occurrence of BPD in children with HMD were analyzed.Results Among 451 children with HMD,BPD occurred in 80 cases,accounting for 17.74%.Logistic regression analysis showed that gestational age ≤ 32 weeks,birth weight ≤ 1.5 kg,complication of intrauterine infection,pulmonary hemorrhage,and mechanical ventilation time>6 d were independent risk factors for the occurrence of BPD in children with HMD(P<0.05,OR>1).Conclusions Gestational age ≤ 32 weeks,birth weight ≤ 1.5 kg,complication of intrauterine infection,pulmonary hemorrhage,and mechanical ventilation time>6 d are independent risk factors for the occurrence of BPD in children with HMD.Clinical treatment and prevention should be strengthened for risk factors to reduce the incidence of BPD.
