3D-HDR-BT治疗鼻咽癌放疗后局部残留患者的疗效

目的评估3D-HDR-BT治疗鼻咽癌根治外照射后局部残留患者的近期和远期疗效。方法对2004-2009年间在本院接受首程根治外照射放疗3个月后局部残留的52例鼻咽癌患者进行腔内和/或咽旁间隙插植3D-HDR-BT治疗,中位剂量12 Gy（5～20 Gy）。采用Kaplan-Meier法计算生存率、无局部复发生存率、无疾病进展率、无远处转移率。结果中位随访时间36个月（17～61个月）,1例远处转移,6例局部复发,所有患者均存活,总生存率100%,1年、2年、3年、5年无局部复发生存率分别为94.2%、92.3%、85.2%、72.5%,1年、2年、3年、5年无疾病进展生存率分别为92.3%、90.4%、81.7%6、6.9%,1年、2年、3年、5年无远处转移生存率均为98.1%。结论 3D-HDR-BT治疗根治外照射后局部残留鼻咽癌患者具有很好疗效。

Clinical Outcomes of 3D-HDR-BT in Treatment for Patients with Locally Persistent Nasopharyngeal Cancer after Radical External Radiotherapy

Objective To evaluation of clinical outcomes of 3DHDR-BT in treatment for patients with locally persistent nasopharyngeal cancer after radical external radiotherapy. Methods Between 2004 to 2009,52 patients with locally persistent nasopharyngeal cancer after radical external radiotherapy were treated with 3D-HDR-BT.Median dose was 12Gy(range:5～20Gy).The Kaplan-Meier method was used to calculate the overall survival rate,the local failure-free survival rate,progression-free survival rate and distant metastasis-free survival rate. Results At a median follow-up of 36 months(range:17～61 months),one patient developed hepatic metastasis and six patients developed local relapse.All patients were still alive,the overall survival rate was 100%.The 1-year,2-year,3-year and 5-year the local failure-free survival rate was 94.2%、92.3%、85.2% and 72.5%,repectively progression-free survival rate was 92.3%、90.4%、81.7% and 66.9%,the distant metastasis-free survival rate all the same as 98.1%. Conclusion 3D-HDR-BT has shown the excellent clinical outcomes to treat patients with locally persistent nasopharyngeal cancer after radical external radiotherapy.
