344例不孕症子宫内膜临床与病理分析

目的通过对不孕症患者子宫内膜的各种病理形态进行回顾性分析,探讨子宫内膜发育情况与不孕症的关系,从而为临床提供诊治依据。方法对344例女性不孕症患者子宫内膜的病理检查结果进行回顾性分析。结果 344例女性不孕症患者原发性不孕240例,继发性不孕104例。其中,149例患者子宫内膜呈增生性反应（占43.31%）,增生性反应患者中73例增生不足（占21.22%）,单纯性子宫内膜增生过长19例（占5.52%）,子宫内膜复杂性增生过长3例（占0.87%）;133例患者子宫内膜呈分泌性反应（占38.66%）,其中分泌不良型子宫内膜74例（占21.51%）,晚分泌期子宫内膜59例（占17.15%）;子宫内膜息肉49例（占14.24%）;子宫内膜炎8例（占2.33%）;内膜结核5例（占1.45%）。结论 344例不孕症患者中,以子宫内膜表现为增生性反应及分泌不良者多见,从而得出不孕症患者的主要病因为卵巢黄体功能不全和卵巢无排卵。

Clinical and Pathologic Analysis of Endometrium in 344 Cases of Infertile Women

Objective To investigate the endometrial quality of infertile women through a variety of pathological retrospective analysis,to explore the relationship between endometrial development situation and infertility so as to provide a basis for clinical diagnosis and treatment. Methods Totally 344 cases of female infertility endometrial biopsy results were retrospectively analyzed. Results Among the 344 female infertility patients,240 patients suffered from primary infertility and 104 suffered from secondary infertility.Totally 149 patients showed a proliferative response of endometrium(43.31%).Among the patients with proliferative response,there were 73 patients with insufficient hyperplasia(21.22%),19 patients with simple endometrial hyperplasia(5.52%),and 3 patients with complex endometrial hyperplasia(0.87%).Totally 133 patients showed endometrial secretory response(38.66%),of which 74 patients showed endometrial secretory response to poor performance(21.51%),59 patients showed late secretory phase endometrium(17.15%),49 patients showed endometrial polyps(14.24%), 8 patients suffered from endometritis(2.33%),and 5 patients suffered from endometrial tuberculosis(1.45%). Conclusions Among the 344 female infertility patients,most of them suffered from proliferative endometrium or endometrial secretory response to poor performance.The results indicate that ovarian dysfunction and ovarian corpus luteum without ovulation are the main pathogeny of infertility.
