腹腔镜与开腹胆囊切除术的临床比较

目的：比较腹腔镜与常规开腹胆囊切除术的临床效果。方法：回顾分析我院2006年3月至2009年3月进行的534例胆囊切除术,其中腹腔镜胆囊切除术（laparoscopic cholecystectomy,LC）组278例,开腹胆囊切除术（open cholecystec-tomy,OC）组256例。比较两组的手术效果及术后并发症发生情况。结果：LC组在手术时间、术中出血量及术后排气时间、术后住院时间均优于OC组（P〈0.05）。LC组术后共发生并发症8例,发生率为2.9%,OC组13例,发生率为5.1%,LC组低于OC组（P〈0.05）。结论：LC较传统的OC具有创伤小、手术时间短、并发症发生率低等优点,值得临床推广和应用。

Comparison of Laparoscopic Cholecystectomy and Open Cholecystectomy

Objective:To compare the clinical effectiveness of laparoscopic cholecystectomy(LC) and open cholecystectomy(OC).Methods:The clinical records of 534 cholecystectomy patients who were in hospital between March in 2006 to March in 2009 were retrospectively analyzed.LC group was 278 patients and OC group was 256 patients.The operation effectiveness and postoperative complications of the two groups were compared.Results:The surgery durations were shorter,the bleeding volumes in operation were less and the postoperative evacuating time was earlier of LC group than those of OC group(P<0.05).There were 8 and 13 patients who had the postoperative complications of LC and OC respectively.And incidence rate of postoperative complications in LC group was 2.9% lower than 5.1% in OC group(P<0.05).Conclusion:LC has advantages of less trauma,shorter operation duration and less postoperative complications than OC,and is worth to apply widespread in clinical.
