站立位动态数字摄影在退变性椎体滑脱症的应用研究

目的探讨站立位动态摄影对退变性椎体滑脱症（DS）的应用价值并进一步研究DS的影像学表现。方法收集2003—2010进行脊椎X线检查的患者2 358例,其中颈椎1 076例,胸椎100例,腰椎1 182例,全组均摄取正位、侧位、过伸中间位、过伸位、过屈中间位、过屈位、过度左侧弯、过度右侧弯等体位、典型滑脱者均做了CT检查。结果全组共发现椎体假性滑脱656例（均经CT证实）,其中颈椎236例,胸椎9例,腰椎411例。正侧位片无滑脱而动态摄影出现者256例,约占39%,其他400例通过动态摄影滑脱均不同程度加重。根据椎体滑脱情况DS可分为一致性前滑脱、阶梯型前滑脱、一致性后滑脱、阶梯型后滑脱、交错性滑脱、左滑脱、右滑脱、旋转并左滑脱、旋转并右滑脱等9种类型。结论站立位动态数字摄影较传统X线摄影能提高对退变性椎体滑脱的X线检出率及确诊率,便于对滑脱进行更细致的分型,且经济、方便、快捷。

Appalication of the dynamic digital radiography of standing location to degeneration spondylolisthesis and its imaging study
