间苯三酚联合同侧俯卧位在胎方位异常分娩中的临床研究

目的:探讨产程活跃期胎方位异常采用药物松弛宫颈并配合孕妇体位干预对纠正胎方位、提高阴道顺产率的效果。方法:产程活跃期以胎方位为枕后位患者,给孕妇应用间苯三酚80mg,并指导孕妇采取与胎儿脊柱同方向的同侧俯卧位的正常初产妇共150例作为观察对象,以孕妇随意卧位150例作为对照组。结果:产程活跃期对胎方位异常给间苯三酚松弛宫颈,并配合孕妇采取同侧俯卧位时,孕妇采用同侧俯卧位组的胎方位纠正率、阴道顺产率均比采取随意卧位的相应的对照组高(P<0.01);持续性枕后位及剖宫产的发生率均比随意卧位相应的对照组低(P<0.01)。结论:在活跃期出现了枕后位产程停滞,及时给松弛宫颈的药品配合孕妇体位干预,采取脊柱同侧卧位可有效的纠正胎方位,提高阴道顺产率,方法简单,孕妇依从性好,易于在任何医院推广使用,有助于提高产科质量。

Clinical study on phloroglucinol combined with ipsilateral prone position in delivery of pregnant women with abnormal fetal position

Objective:To explore the effects of drug use in order to relax uterine cervix and intervention on body position of pregnant women during active phase of labor stage on correcting fetal position and improving the rate of vaginal spontaneous delivery in pregnant women with abnormal fetal position.Methods:For the pregnant women with occipitoposterior position, 80 mg phloroglucinol was given, 150 normal primiparas adopting ipsilateral prone position were selected as observation subjects, 150 pregnant women adopting random position were selected as control group.Results:The correct rate of fetal position, rate of vaginal spontaneous delivery in observation group were significantly higher than those in control group (P<0.01); the incidence of persist occipitoposterior position and rate of cesarean section in observation group were significantly lower than those in control group (P<0.01).Conclusion:Timely drug use to relax uterine cervix, intervention on body position of pregnant women and adopting ipsilateral prone position may correct abnormal fetal position and improve the rate of vaginal spontaneous delivery, the methods are simple and with good dependence, which can be extended easily in any hospitals and contribute to improve the quality of obstetrical department.
