鼻咽癌动态调强剂量学比较

摘    要:目的:探讨在相同的优化参数下鼻咽癌动态调强不同野数间的剂量学差异。方法:利用ECLIPSE7.3治疗计划系统对30例鼻咽癌调强治疗的患者,采用相同的优化参数,每例病人均采用5,7,9野进行优化,比较其对靶区及保护器官上的剂量学差异及低剂量区体积的比较。剂量学评估参数:PTV最大剂量(PTVDmax)、PTV平均剂量(PTVDmean)、PTV最小剂量(PTVDmin)、剂量体积直方图(DVH)、M-C(机器跳数)、低剂量受照体积等数据进行评价。结果:(1)PTVDmax:5野高于7、9野比较有统计学意义(P〈0.05),而7、9野间比较(P〉0.05)无统计学意义。(2)MU:9野高于5、7野比较差异有统计学意义(P〈0.01),而5、7野之间差异无统计学意义(P〉0.05);(3)BD10-BD20剂量区比较:5野高于7、9组(P〈0.05),7野和9野间比较(P〉0.05);BD10-BD30方面各组间比较(P〉0.05)无统计学意义。结论:鼻咽癌动态调强不同野数的治疗计划均能为临床提供较满意的剂量学分布,9野方式略优于其他组,但MU较多:5野BD10-BD20剂量受照体积较7、9野大;7,9野两组间无差异。

Abstract:Objective: To investigate the characteristics of sliding window and step and shoot dosimetry difference for nasopharynx cancer IMRT. Method: Three IMRT plans were designed for each of 30 patients with Nasopharynx Carcinoma with Eclipse 7.3 TPS, compared with in the dosimetry difference. Dosimetry parameters: PTVDmax ,PTVDmean ,PTVDmin, DVH, MU, Data evaluation. Results: ( 1 )PTV Dmax :5Field more than 7,9 Field (P〈0.05), 7,9 Field compare (P〉0.05). (2) MU : 9Field more than 5,7Field (P〈0.01), 5Field vs 7Field (P〉0.05) ; (3)BD10-BD20: 5Field more than 7,9 Field (P〈0.05), 7Field vs 9 Field compare (P〉0.05) ;BD10-BD30, apiece group compare (P〉0.05) o Conclusions: The nasopharynx cancer IMRT plan to be able to provide the more satisfactory dosimetry distribution for clinical, 9Field slightly better than other group,but need more MU complete the treatment. 5Field BD10-BD20 dose volume greater than 7,9 Field, 7,9 Field two groups does not have the difference.
