补肝汤加味联合复方甘草酸苷胶囊治疗慢性乙型病毒性肝炎临床观察

目的:观察补肝汤加味联合复方甘草酸苷胶囊治疗慢性乙型病毒性肝炎的疗效。方法:将慢性乙型病毒性肝炎患者94例随机分为对照组和治疗组各47例。对照组患者给予复方甘草酸苷胶囊口服,3粒/次,3次/d;治疗组患者在此基础上加用补肝汤加味口服,2组均连续治疗6个月。治疗前后检测肝功能指标,比较2组患者乙型肝炎病毒(HBV)DNA、乙型肝炎E抗原(HBe Ag)转阴率;通过检测T淋巴细胞水平观察免疫功能的改善情况,并观察临床疗效。结果:治疗组的有效率为91.5%,高于对照组的72.3%,差异有统计学意义(P0.05);治疗组患者的肝功能指标改善优于对照组,其ALT、AST、TBil水平较对照组降低,而ALB水平升高,差异有统计学意义(P0.05);治疗组患者的HBV-DNA、HBe Ag转阴率高于对照组,差异有统计学意义(P0.05);治疗组患者与机体免疫力相关的T细胞亚群CD4+、CD4+/CD8+水平明显高于对照组,而CD8+水平低于对照组,差异有统计学意义(P0.05)。结论:补肝汤加味联合复方甘草酸苷胶囊治疗慢性乙型病毒性肝炎疗效确切,具有良好的抗病毒作用,其机制与促进肝细胞修复再生、提高机体免疫力密切相关。

Clinical Observation on Modified BuGan Tang Combined with Compound Glycyrrhizin Capsules in the Treatment of Chronic Viral Hepatitis B

Objective: To observe clinical effects of modifiedBuGanTangcombined with compound glycyrrhizin capsules in treating chronic viral hepatitis B. Methods: All 94 patients were randomized into the control group and the treatment group, 47 cases each group. The patients in the control group took compound glycyrrhizin capsules orally, three pills each time, three times per day; the patients in the treatment group took modifiedBuGanTang, both groups were treated for six months consecutively. HBV-DNA and HBeAg negative conversion rates of both groups were com-pared by detecting the indexes of liver functions before and after treating; the improvement of the immunity was ob-served by detecting the levels of T lymphocytes, and clinical effects were observed. Results: The effective rate of the treatment group was 91.5%, higher than 72.3% of the control group evidently, and the difference had statistical meaning (P<0.05); the treatment group was superior to the control group in improving the indexes of liver function, lower than the control group in the levels of ALT, AST, TBil obviously, while the levels of ALB increased, and the difference pre-sented statistical meaning(P<0.05); the treatment group was higher than the control group in the HBV-DNA and HBeAg negative conversion rate and the difference showed statistical meaning(P<0.05); the treatment group was high-er than the control group in the levels of T lymphocytes subgroup CD4+, CD4+/CD8+ related to body immunity obvious-ly, lower than the control group in the level of CD8+ evidently, and the difference had statistical meaning(P<0.05). Conclusion: ModifiedBuGanTangcombined with compound glycyrrhizin capsules in treating chronic viral hepatitis B could obtain definite effects, it shows better antiviral effects, the mechanism is closely related to promote the recovery and regeneration of liver cells, improve body immunity.
