彩色多普勒超声对绝经后宫颈癌血流变化及其意义的探讨

目的　为了探讨绝经后宫颈癌血流动力学的变化及其临床意义。方法　对６５ 例绝经后的宫颈癌和４０ 例绝经后的正常妇女进行了ＣＤＦＩ检查。结果　宫颈癌患者宫颈处见丰富的血管回声，子宫动脉收缩期的血流速度为２１ ±２１４ｃｍ／ｓ，较正常组（１０ ±１９５ ｃｍ／ｓ） 高。两者比较有显著性差异（ Ｐ＜ ００１） 。宫颈癌患者子宫动脉的阻力指数０４ ±００５６ ，较正常组０７ ±００７４ 低。两者比较有显著性差异（ Ｐ＜ ００１） 。结论　绝经后宫颈癌导致子宫动脉产生高速低阻的血流和宫颈处血管数目增加。随着肿瘤内血管数目的增加，肿瘤宫旁浸润和淋巴结转移的发生率逐渐增加

A Comparative Study of Color Doppler Ultrasound on Hemodynamics of Postmenopausal Cervical Carcinoma and Its Clinical Significance

Objective To determine hemodynamic changes in postmenopausal cervical carcinoma and its clinical significance.Methods Color Doppler examination was performed on 65 patients with postmenopausal cervical carcinoma and 40 postmenopausal healthy women.Results Rich angiogenesis was detected by color Doppler ultrasound in postmenopausal cervical carcinoma.There was a significant difference of the peak systolic velocity of the uterine artery between the patients and the contrast group(PS 21±2 14 cm/s vs 10±1 95 cm/s, P <0 01).The peak systolic velocity of the postmenopausal cervical carcinoma was higher than that of the postmenopausal healthy women.However,the corresponding arterial resistance index of the patient was lower than the contrast group(RI 0 4±0 056 vs 0 7±0 074, P <0 01).There was also significant difference on the resistance index between these two groups( P <0 01).Conclusion The postmenopausal cervical carcinoma exhibited higher velocity blood flow with lower resistance index in uterin artery and more vascularity in the uterine cervix than healthy women.The intratumoral neovascularization by color Doppler ultrasound correlated well with a higner incidence of metastasis around the cervix and pelvic lymph node metastasis in postmenopausal cervical carcinoma.
