纳络酮治疗急性重型颅脑损伤患者血浆内皮素含量变化及其临床意义

摘    要:目的 探讨纳络酮治疗急性重型颅脑损伤患者血浆内皮素(ET)含量变化及其临床意义,评价纳络酮治疗急性重型颅脑损伤的意义。方法 观察了52例纳洛酮治疗急性重型颅脑损伤患者血浆ET含量在治疗前1d、治疗后3、8d及2周的变化,并且以46例颅脑损伤患者按常规治疗作对照组。结果 颅脑损伤24h内两组患者血浆内皮素较正常人均明显升高(P〈0.05),伤后第3天、第8天两治疗组血浆内皮素较治疗前均明显下降(P〈0.05),但纳络酮治疗组低于常规治疗组(P〈0.05),治疗2周后两组ET水平基本恢复到正常水平。结论 急性重型颅脑损伤患者血浆内皮素含量明显升高,与病情轻重相关,纳络酮治疗较常规治疗能更迅速地降低血浆ET水平。

Abstract:Objective To explore the changes of plasma endorphin content in patients with acute severe craniocerebral injury treated with naloxone and the clinical significance.Methods Changes of plasma endorphin contents in 52 cases of naloxone treated craniocerebral injury and 46 cases of routinly treated craniocerebral injury were observed at 24 h,8 day,and 2 weeks after traumatic brain injury.Results Levels of plasma endorphin were all increased (P< 0.01) in both groups before treatment as compared with normal controls.The increased endorphin levels were dropped significantly on the 3rd day and 8th day after treatment in both group (P< 0.01) as compared with those before treatment,but the levels of ET were lower obviously in naloxone group than in routine treatment group (P< 0.05). The plasma endorphin levels in both groups were returned to the normal level 2 weeks later.Conclusion The plasma endorphin levels were increased significantly after sever cranioerebral trauma and correlated with severity of patient's condition.Naloxone treatment could rapidly decrease the plasma endorphin levels as compared with routine treatment.
