前外侧入路人工股骨头置换术前牵引临床疗效分析

目的 探讨前外侧入路人工股头转换手术前牵引对手术及疗效的影响。方法 回顾性分析97例外侧入路人工股骨头转换手术治疗的临床资料。结果 97例经髋关节前外侧入路手术，其中86例术前采用下肢皮牵引带牵引，11例术前采用骨牵引，术中复位均较容易；术后优良占93．1％，很好占4．2％，好占2．7％，10年内无后脱位和修正。结论 前外侧入路人工股骨头转换术前采用下肢皮牵引比骨牵引引更理想；股骨距（即股骨颈内侧皮质）残留长度和术后早期功能锻炼是决定疗效的关键。

Preoperative traction in artificial femoral head replacement from anterolateral approach：A clinical curative effect analysis

Objective To discuss the curative effect of preoperative traction in artificial femoral head replacement from anterolateral approach.Methods Ninety-seven cases underwent artificial femoral head replacement were studied retrospectively.Results Ninety-seven cases were operated from anterolateral approach of hip joint.Among them,86 cases underwent the skin traction in lower extremity and 11 cases,skeletal traction preoperatively.The intraoperative reposition were all quite easier.The postoperative effect was excellent in 93.1%,better in 4.2% and good in 2.7% of the cases.No backward dislocation and restoration was found within 10 years.Conclusion Preoperative skin traction is more satisfactory than skeletal traction in artificial femoral head replacement from anterolateral approach.Residual length of femur range and early functional exercise would be the key factors for the curative effect.
