结肠直肠癌P27kip1蛋白的表达及临床意义

目的：探讨结肠直肠癌肿瘤组织中P27^kip1蛋白的表达情况与预后关系。方法：通过免疫组织化学染色法检测2002年12月至2004年12月于本院接受治疗的212位结肠直肠癌病人肿瘤组织标本中P27^kip1蛋白的表达，同时对其中接受根治术的202例病人进行随访．分析P27^kip1蛋白的表达与病人性别、年龄、肿瘤生长部位、大小、病理类型、分期及根治性手术后肿瘤发生转移、复发的关系。结果：212例结肠直肠肿瘤组织中P27^kip1蛋白染色阳性表达者131例．占61．8％。P27^kip1蛋白阳性表达与肿瘤直径呈负相关（P=0．045）：P27kip1。蛋白阳性表达程度的高低与肿瘤复发率、死亡率呈负相关（P〈0．001）。行半结肠癌的P27州蛋白阳性表达与肿瘤淋巴转移情况呈负相关（P=O．003）。但P27^kip1蛋白表达与病人性别、年龄、肿瘤生长部位、病理类型、分期及有无转移均无显著相关性（P〉0．05）、结论：结肠直肠癌肿瘤组织中P27^kip1蛋白的表达情况可以评估结肠直肠癌手术治疗的预后情况，具重要的临床参考价值。

P27kip1 protein expression in colorectal cancer and its clinical significance

Objectives To detect the expression of P27kip1 protein in colorectal cancer tissues, and to explore its relationship with the biological behaviors of the tumor and the prognosis of the cases involved. Methods From Dec. 2002 through Dec. 2004, 212 cases of colorectal cancer were admitted, and of them, 202 cases underwent radical resection. P27kip1-expression was determined by immunohistochemistry. The correlation between P27kip1-expression and the clinical outcome ,and the clinicopathologic features, such as gender, age, tumor location, tumor size, tumor type, TNM staging and recurrence status after curative surgery were investigated. Results The percentage of P27kip1-expression of the series was 61.8% (131/212). The expression of P27kip1 protein was inversely correlated with the tumor diameter (P=0.045), recurrence rate and mortality rate of the cases involved (P<0.001). In ascending colon cancer cases, P27kip1-expression was inversely related with nodal status of lymph node metastasis (P=0.003). No relationship was found between P27kip1 protein expression and the other parameters such as gender, age, tumor location, type, staging and metastasis. Conclusions P27kip1 protein might serve as an important prognostic predictor of the colorectal cancer cases.
