| 摘 要： | 目的 探讨重症急性胰腺炎(SAP)合并呼吸衰竭患者腹腔感染发生的危险因素及病原菌流行情况,为SAP的治疗提供依据.方法 回顾性分析2003年10月-2010年9月我院ICU收治的58例SAP合并呼吸衰竭患者的临床资料,根据临床表现及腹腔病原菌培养结果,分为腹腔感染(IAI)组及非感染(NI)组,比较两组急性生理学和慢性健康(APACHE)II评分、机械通气(MV)时间、腹内压、多器官功能障碍综合征(MODS)数≥3、外科干预率、连续肾脏替代治疗(CRRT)率及胆源性胰腺炎.调查IAI组病原菌流行情况.结果 SAP合并呼吸衰竭患者腹腔感染率为51.72%(30/58).MV时间、腹内压、外科干预是腹腔感染的独立危险因素.病原菌前10 d以革兰阴性杆菌为主,之后革兰阳性菌及真菌逐渐增加.结论 MV时间、腹内压及外科干预与SAP腹腔感染密切相关.SAP合并呼吸衰竭患者腹腔感染病原菌流行随病程的不同而变化.
