下腔静脉滤器临床应用体会

摘    要:目的探讨下腔静脉滤器(IVCF)置入术在下肢深静脉血栓(DVT)综合治疗中预防肺栓塞的临床应用价值。方法选自我院住院治疗的32例下肢DVT患者经股静脉途径行下腔静脉滤器置入术,以防止肺动脉栓塞。IVCF置入后同时行抗凝、溶栓治疗。结果 32例下腔静脉滤器均一次性置入成功,技术成功率100%。32例DVT行IVCF患者无一例发生致命性肺栓塞。所有患者下肢症状明显好转。本组2例出现滤器倾斜,但倾斜角度小于15度。1例发生下移,但未进入髂静脉内,不影响滤过效果。未发生下腔静脉穿孔、阻塞等并发症。结论经皮下腔静脉滤器置入术,置入方法简单、安全、并发症少,可有效预防肺动脉栓塞的发生。

Abstract:Objective To discuss the value of clinical application ofinferior vena cava filter(IVCF) placement in deep vein thrombosis(DVT) to prevent pulmonary embolism.Methods Thirty-two cases with lower DVT in the hospital were enrolled and underwent inferior vena cava filter placement to prevent pulmonary embolism.After IVCF placement anticoagulation and thrombolytic therapy was done simultaneously.Results Inferior vena cava filter placement was successful in all patients.Technical success rate was 100%.No fatal pulmonary embolism occurred.Lower symptom was improved obviously in all patients.The filter tilted in 2 cases,but the angle is less than 15 degrees.One filter moved down,but it did not enter iliac veins.There were no inferior vena cava perforation and block complications.Conclusion It can effectively prevent pulmonary embolism with subcutaneous inferior vena cava filters placement,and the placement method is simple,safe with less complications.
