老年糖尿病患者颈动脉狭窄程度与脑梗死的相关性

目的探讨老年糖尿病脑梗死患者颈外动脉狭窄的特点。方法采用HP-5500彩色多普勒超声仪分别对脑梗死组和对照组颈外动脉进行监测,观察颈动脉内—中膜厚度(IMT),管腔有无狭窄及狭窄的程度,有无斑块形成及斑块大小和血管解剖学形态。结果脑梗死组患者颈动脉狭窄出现的比例明显高于对照组(P<0.001);脑梗死组患者颈动脉狭窄的部位以颈动脉起始部和颈动脉膨大部最明显(P<0.01),脑梗死组患者颈动脉狭窄以梗死侧为高,其比例明显高于非梗死侧(P<0.001)。结论老年糖尿病脑梗死患者颈动脉狭窄与脑梗死的发生呈正相关,与颈动脉内膜厚度呈正比,梗死部位与颈动脉狭窄的严重程度具有同侧性,梗死发生与否与狭窄有关,与狭窄部位相关性不大,且狭窄部位多发生在颈内动脉起始部位和膨大部位。

Relationship between carotid arterial stenosis degree and cerebral infarction in old aged diabetic patients

objective It is to discuss the character of external carotid artery stenosis in old aged diabetic patients with cerebral infarction.Methods The lumen diameter,intimacy-media thickness(IMT),plaque and artery stenosis severity of external carotid artery in cerebral infarction group and control group were observed with HP-5500 color Doppler ultrasound.Results The frequency of carotid arterial stenosis in cerebral infarction group was obviously higher than that in control group(P<0.001).In most cerebral infarction patients,the stenosis was seen at the start part and the intumescence part of carotid artery(P<0.01). The occurrence rate of carotid arterial stenosis in infarct side was higher than that in non-infarct side(P<0.001).Conclusion Cerebral infarction in old aged diabetic patients is closely associated with carotid arterial stenosis.Its proportion is direct with the carotid artery IMT.There was significant positive correlation between infarction part and the same side of carotid arterial stenosis.In most cerebral infarction patients,the stenosis was seen at the start part and intumescence part of internal carotid artery.
