加压螺纹钉内固定治疗股骨颈骨折的力学测试和临床应用

作者设计一种新型治疗股骨颈骨折的加压螺纹钉。将其固定于股骨颈骨折标本,在ZDM—500型试验机上行静力学测定,其骨折部3mm张开移位时载荷及最大载荷为2641与3072N,经临床应用80例,27例采用闭合复位电视X线透视下穿钉内固定术;53例行开放复位加压螺纹钉内固定与股外侧肌骨瓣移植术;64例得到随访,随访时间平均5年8个月。骨折愈合率为93.8％,股骨头缺血坏死率为6.2％。结果显示,自制加压螺纹钉设计合理,具有自攻能力,固定可靠,操作简单,符合生物力学原理。

Treatment of Femoral Neck Fracture with a New Compression-screw Internal Fixation

The authors designed a new compression-screw for the treatment of femoral neck fracture. After specimens of femoral neck fractures were fixated by the compression-screw,tests of statics were performed on the ZDM-500 tesing machine. The mean of maximal load was 3072N and load of 3mm displacement at the osteotomy site was 2641N. 80 cases of femoral neck fracture had been treated by compression-screw. 27 cases were treated with internal fixation of compression-screw through a small incision. 53 cases were treated with vastus lateral muscle-pedicle-bone graft and compressionc-screw internal fixation. 64 patients were fol-lowed-up for 68 months in average. The result indicated that the rate of healing was 93. 8 percent and avascular necosis of the femoral head occurred in 4 patients. The new compression-screw has the ability of tapping and does less damage to the bone. The operation is simple to perform with the use of compression-screw to fixate femoral neck fracture.
