异丙酚复合地佐辛在宫腔镜手术麻醉中的疗效评价

摘    要:目的评价异丙酚复合地佐辛用于宫腔镜手术麻醉的效果和安全性。方法拟在静脉全麻下行宫腔镜手术的患者90例,年龄25~60岁,体重45~65kg,ASA分级Ⅰ级或Ⅱ级。随机分为3组(各组n=30):单纯异丙酚组(P组),异丙酚复合0.001mg/kg芬太尼组(PF组),异丙酚复合0.1mg/kg地佐辛组(PD组)。记录3组手术时间、异丙酚总用量、发生呼吸抑制的例数、出现体动的例数、麻醉苏醒时间,观察不良反应发生情况(头晕,恶心,呕吐)并询问术后有无宫缩痛。结果与P组比较,PD组和PF组异丙酚总用量减少(P〈0.05),术中出现体动的例数减少(P〈0.05),术后宫缩痛的发生率降低(P〈0.05),与PF组比较,PD组呼吸抑制的发生率降低(P〈0.05)。结论异丙酚复合地佐辛用于宫腔镜手术麻醉效果确切,异丙酚用量减少,术中体动及术后宫缩痛的发生率降低,呼吸抑制较芬太尼轻,值得临床推广使用。

Abstract:Objective To investigate the feasibility and safety of dezocine combined with propofol for the surgery anesethesia of hysteroscope.Methods 90 ASA Ⅰ ~ Ⅱ patients aged 25 ~60 year weighing 45 ~65kg undergoing hysteroscopic surgery were divided into 3 groups(n=30,each):propofol group(group P),propofol combined with fentanyl group(group PF),propofol combined with dezocine group(group PD).To record the total dosage of propofol,respiratory depression,cases body movement,recovery time of anesthesia,the incidence of adverse reactions(dizziness,nausea,vomiting) and uterine contraction pain after surgery.Results Compared with group P,dezocine significantly decreased the total dosage of propofol(P〈0.05),reduced body moving rate and uterine contraction pain(P〈0.05),compared with group PF,the incidence of respiratory depression in group PD was significantly lower(P〈0.05).Conclusion Propofol combined with dezocine is an effective method for hysteroscopy operation.Propofol dosage reduction,the incidence reduced of intraoperative body dynamic and intraoperative reuterine contraction pain,compared with fentanyl respiratory inhibition is lighter.
