摘    要:研究幽门螺杆菌(H.pylori)感染胃癌得胃粘膜病变中抑癌基因p53、p16和关键性凋亡调节基因bc1-2蛋白的表达,进一步探讨H.pylori在胃癌发生、发展过程中作用的分子机制。方法:胃镜检查及外科手术中取40例胃癌患者的癌组织和癌旁2 cm处组织各2块,石蜡包埋,切片HE染色作病理诊断及免疫组化检查p53、p16及bc1-2蛋白的表达。H.pylori阳性由快速尿素酶试验结合病理染色/^1

Abstract:Background/Aims: To investigate the expression of tumor suppressive gene and regulatory geneof apoptosis in gastric mucosal lesions of gastric cancer patients with Helicobacter pylori (H. pylori) in-fection. Methods: 40 cases of gastric carcinoma patients underwent endoscopy and surgical resection,samples were taken from tumor site and paracancerous tissues. ABC immunohistochemical stainingwas used to detect the expression of p53, p16, bcl-2 proteins. H. pylori was determined by rapid ureasetest combined with pathological staining/14C-urea breath test. Results: There was no significant difference in the positive rate of p53 expression of gastric carcinoma between H. pylori-positive and H.pylori-negative group (P>0.05). The positive rate and staining intensity of p16 expression in patientswith chronic gastritis or intestinal metaplasia with H. Pylori-positive were significantly lower thanthose with H. pylori-negative (positive rate, P< 0.05; staining intensity, P< 0.001 and P< 0.01). In con-trast to p16, bcl-2 expression in patients with intestinal metaplasia with H. pylori-positive was signifi-cantly higher than that with H. pylori-negative (P<0.05). Conclusions: p16 or bcl-2 gene expressionalteration might play a role in the early development/promotion of gastric carcinoma, which is as-sociated with H. Pylori infection. It seems that H. pylori infection is not related to p53 gene alterationbecause p53 gene overexpression is a relatively late event in gastric tumorigenesis.
