隐匿性脑血管畸形的CT、MRI诊断

目的 探讨隐匿性脑血管畸形的CT，MRI表现特征及鉴别。方法 对15例随访5年以上的隐匿性脑血管畸形的CT、MRI进行回顾性分析。结果 15例患中，病灶位于左侧额颞区9例，右侧额叶3例，右侧额顶2例，左侧枕叶l例。CT平扫病灶呈等高混合密度，病灶内可见线条状钙化。MRI平扫病灶于T1WI呈等信号，T2WI呈低、等和高混合信号。增强后CT扫描4例见轻度非均匀强化，余ll例未见强化；全部病灶MRI增强扫描未见强化。全部病灶无占位效应硬病灶周围水肿，10例邻近病灶的脑沟增宽。结论 隐匿性脑血管畸形因血栓形成、血栓钙化等致病灶的密度、信号不均匀，无占位表现和随访无增大是其表现特征。

CT and MRI diagnosis of cryptical cerebral vascular malformations

ObjectiveTo investigate CT and MRI features of cryptical cerebral vascular malformations.MethodsCTand MRI data of 5 patients with cryptical cerebral vascular malformations were analyzed retrospectively. All patients werefollowed-up more than 5 years.ResultsOf 15 cases, the lesions arose from the left frontoparietal regions in 9 cases, theright front lobe in 3 cases, the right frontoparietal regions in 2 cases, the left occipital lobe in 1 case. The lesions were mixediso- and hyperdensity on CT plain scanning. The curveline calcification were found in the lesions. The lesions were isointensityon T1-weighted and heterogeneous signals on T2-weighted MR imaging. After injection of contrast medium, 4 cases showedmild heterogeneous enhancement on CT contrast scanning and the remained 11 cases were not enhanced. All cases showed noenhancement on T1-weighted MR imaging. Space-occupying effects and peritumoral edema were not seen in the all patients.Gyral enlargement were seen in 10 patients. ConclusionsBecause of thrombosis and its calcification, the density andsignal of the lesions are heterogeneous. The characteristics of cryptical cerebral vascular malformations are no space-occupy-ing effectsperitumoral edema and no changing in the period of following-up.
