阻塞性睡眠呼吸暂停低通气综合征对脑梗死预后影响

摘    要:目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)对脑梗死预后的影响。方法70例急性脑梗死患者,经多导睡眠图检查后,合并OSAHS脑梗死46例(65.7%)。60例患者完成随访研究,其中OSAHS组40例、非OSAHS组20例。OSAHS组据睡眠呼吸暂停低通气指数(apnea—hypopnea index,AHI)的不同分为3组(轻度OSAHS组22例、中度OSAHS组11例、重度OSAHS组7例)。随访观察2组入院时、1个月、3个月、6个月神经功能缺损程度、日常生活能力和残障量表。结果合并OSAHS的脑梗死患者.神经功能恢复滞后,日常生活能力低、残障程度高,且与OSAHS的严重程度有关。结论阻塞性睡眠呼吸暂停低通气综合征影响脑梗死患者的预后,临床工作中应重视脑梗死患者的OSAHS问题。

Abstract:Objective To investigate the influence of obstructive sleep apnea-hypopnea syndrome(OSAHS) in the prognosis of cerebral infarction. Methods Seventy patients (46 cerebral infartion with OSAHS,65.7%) were selected. And 60 of them had finished the follow-up investigation. There were 40 patients in the OSAHS group which included 22 mild cases, 11 moderate cases and 7 sever cases,and there were 20 patients in non-OSAHS group. The National Institutes of Health Stroke Scale,Barthel index and Modifial Rankin Scale were observed at admission, 1 month, 3 months, 6 months through follow-up visit. Results Cerebral infarction patients with OSAHS often showed delayed recovery of neural function,low activity of daily living and high degree of disability which were related to the severity of OSAHS. Conclusion OSAHS could affect the prognosis of cerebral infarction.
