摘    要:目的 研究肝素抗凝对经股静脉植入临时心脏起搏器深静脉血栓 (DVT)的预防作用。方法 4 8例经股静脉植入临时心脏起搏器病人接受 2种剂量肝素抗凝 ,评价不同抗凝方案对于防止发生深静脉血栓的安全性和有效性。结果 所有病人穿刺对侧下肢均无血栓性闭塞 ,病人或临床特征与DVT的危险性增加无明显相关。治疗剂量肝素应用是DVT预见性唯一有显著意义的独立因素 (P <0 0 0 1)。结论 经股静脉植入临时心脏起搏器与DVT的危险呈明显相关 ,在实施经股静脉植入临时起搏器给以肝素预防治疗对于降低DVT的危险率是安全有效的方法。

Abstract:Objective To discuss the preventive effect of anticoagulation heparin on deep venous thrombosis(DVT) associated with temporary transvenous pacemakers.Methods Fourty-eight patients with temporary pacemarkers received anticoagulation heparin.They received either prophylactic heparin or therapeutic heparin.The efficacy and safety of different heparin regimes in preventing DVT were evaluated.Results No thrombus was noted in the contralateral limb in the patients.No patients or clinical charcteristic was associated with an increased risk of DVT.The applicaton of therapeutic heparin was the only independent factor which was a significant negative predictor of DVT(P<0.001).Conclusion Temporary transvenous pacemakers are associated with the risk of DVT.Therapeutic anticoagulation heparin is safe and effective in reducing the risk of DVT when temporary pacemarker is employed transfemoral vein.
