影响留置针穿刺成功的原因分析及应对措施

目的探讨分析影响留置针穿刺成功的原因,并根据问题提出有效的应对措施。方法回顾性2012年1月至2013年4月在我院进行静脉留置针穿刺的1000例患者的临床资料,整理、分析留置针穿刺失败的原因。结果 1000例患者中留置针穿刺仅1次成功者880例,成功率为88%,需要多次穿刺成功者120例,主要从操作环境因素、患者自身因素、护士因素以及血管状况等方面进行分析,总结穿刺失败的原因主要是由于:1针头刺入过深穿透血管,导致局部渗血肿胀。2针头刺入血管过浅,导致送外套管时发生滑脱移位。3针头未刺入血管,导致没有回血。4送外套管难度大,静脉瓣阻挡了外套管。结论首先护理人员应加强自身素质,提高留置针操作的专业技能,穿刺时选择合适的血管,为患者营造一种温馨、和谐的氛围,尽可能提高一次穿刺成功率,减轻患者的痛苦。

Effect the Success of Indwelling Catheter Cause Analysis and Countermeasures

Objective To analyze the effects of indwelling needle puncture success reason, and according to the problems we put forward some effective counter measures. Methods Retrospective in 2012 January-2013 April, 1000 cases of venous indwelling needle in patients in our hospital were sorted, analyzed of the causes of indwelling needle puncture failure. Results 880 cases of 1000 patients with indwelling needle puncture only one time winner, the success rate was 88%, 120 cases need to repeatedly puncture success, mainly carries on the analysis from the aspects of environmental factors, the nurse factors, vascular factors, we summarized the main reason of puncture failure： 1the needle penetration of blood vessels is too deeply, leading to localoozing swelling. 2The needle piercing the vessel is too shallow, resulting inspondylolisthesis shift occurred to the outer tube. 3The needle is not penetrate into the blood vessels, resulting in no blood. 4The outer tubefeeding difficulty, venous valve block of the outer sleeve tube. Conclusion The nursing staff should improve their own quality and the professional skills of indwelling needle puncture operation, selection of suitable vessels, and create a warm, harmonious atmosphere for the patient, as far as possible to improve the success rate of first puncture, alleviate the suffering of patients.
