脊髓型颈椎病前路手术的疗效及影响因素分析

摘    要:目的探讨前路手术对脊髓型颈椎病的治疗效果及影响手术效果的相关因素。方法回顾性分析52例行前路手术的脊髓型颈椎病患者的临床资料,观察其脊髓功能术后改善率,并根据年龄、病程、术前评分进行分组,观察年龄、病程、术前功能评分对手术治疗效果的影响。结果所有患者的神经功能均得到不同程度的改善,其中术后改善优良率为80.77%。年龄轻者、病程短者、术前功能评分高者手术治疗效果好。结论颈前路植骨融合内固定手术治疗脊髓型颈椎病疗效确切。年龄、病程、术前脊髓功能评分是影响早期手术治疗效果的重要因素。

Abstract:Objective To investigate the clinical effect of anterior approach surgery for cervical spondylotic myelopathy and the influential factor. Methods The clinical data of 52 patients of cervical spondylotic myelopathy with anterior operation were retrospectively analyzed. The improvement rates of the spinal cord function after operation were investigated. The patients were divided into groups according to the age, course of disease, and preoperative score. The effect of age, course of disease and preoperative function score on the clinical efficacy were observed. Results All the patients with nerve function were improved to different degrees. The excellent and good rate was 79.27%. The clinical effects were better for patients of younger age, short duration, and higher preoperative functional scores. Conclusion Cervical anterior decompression and bone graft fusion internal fixation is effective for treating cervical spondylotic myelopathy. Age, disease duration, preoperative spinal cord function scores are important factors affecting the clinical efficacy.
