抑郁症与躁狂症患者心肌酶谱比较分析

摘    要:目的探讨抑郁症患者与躁狂症患者的心肌酶谱差异特征,分析其相关的影响因素。方法筛选40例汉族抑郁症患者和38例躁狂症患者纳入研究对象,采用汉密尔顿抑郁量表(HAMD)、焦虑量表(HAMA)、躁狂评定量表(BRMS)评定临床症状严重程度,检测心肌酶五项。采用SPSS13.0软件统计分析。结果入院时抑郁症患者组与躁狂症患者组心肌酶五项中磷酸肌酸激酶(CK)(χ2=4.863,P<0.01)、磷酸肌酸激酶同工酶(CK-MB)(χ2=8.642,P<0.01)、乳酸脱氢酶(LDH)(χ2=3.297,P<0.05)比较差异有统计学意义,羟丁酸脱氢酶(α-HBDH)以及同型半胱氨酸(HCY)在两组间没有阳性发现。躁狂症患者BRMS评定临床症状严重程度与心肌酶五项中CK(r=0.429,P<0.01)、CK-MB(r=0.283,P<0.05)、LDH(r=0.214,P<0.05)呈正相关性。结论躁狂症患者心肌酶三项指标明显高于抑郁症患者,且躁狂症患者临床严重程度与心肌酶五项呈正相关。

Abstract:Objective To analyze the myocardium enzymes spectrum in 40 manic patients (group A) and 38 depressive patients (group B), and analyze the influence factors. Methods Thirty eight manic patients and 40 depressive patients were enrolled in this study. The clinical symptoms of the patients were assessed by HAMD, HAMA, BRMS. Meanwhile, the venous blood samples were collected for assay of myocardium enzymes spectrum. Results The two groups showed statistically significant difference in creative kinase (CK, χ2=4.863,P<0.01), creatine kinase Isoenzyme-MB (CK-MB, χ2=8.642, P<0.01), and lactate dehydrogenase (LDH, χ2=3.297, P<0.05), with no statistically significant difference inα-Hydroxybutyrate dehydrogenase (α-HBDH) and homocysteine (HCY). BRMS scores in the manic patients were highly correlated with CK (r=0.429, P<0.01), CK-MB (r=0.283, P<0.01), LDH (r=0.214, P<0.05). Conclusion The manic patients had higher levels than depressive patients in the myocardium enzymes spectrum.
