分阶段使用外固定架和石膏外固定治疗胫腓骨开放性骨折的临床疗效观察

目的探讨分阶段使用外固定架和石膏外固定治疗胫腓骨开放性骨折的临床疗效。方法选取本院2008年1月至2011年6月收治的胫腓骨开放性骨折患者94例,随机分为两组,仅使用石膏外固定治疗的患者47例为对照组,分阶段使用外固定架和石膏外固定治疗的患者47例为观察组,比较两组患者的各项临床指标。结果观察组住院时间(12.8±6.9)d]和骨折愈合时间(16.4±3.8)周]明显短于对照组(17.7±8.3)d和(19.6±4.1)周],观察组并发症发生率(4.3%)明显低于对照组(19.1%),观察组术后总优良率(97.9%)明显高于对照组(83.0%),差异均有统计学意义(P<0.05)。结论分阶段使用外固定架和石膏外固定治疗胫腓骨开放性骨折愈合时间较早,利于患者进行早期的功能恢复性锻炼,可以大幅降低患者并发症的发生率,是一种安全有效的治疗手段,值得临床推广使用。

Clinical observation of staged external fixation and plaster external fixation in the treatment of open fractures of tibia and fibula

Objective To investigate clinical efficacy of staged external fixation and plaster external fixation in the treatment of open fractures of tibia and fibula. Methods Ninety-four patients with open fractures of tibia and fibula were selected in the hospital from January 2008 to June 2011 and randomly divided into two groups. Patients in the control group (n=47) only used plaster external fixation for treatment, while those in the study group (n=47) were treated by staged external fixation and plaster external fixation. The clinical indicators of two groups were compared. Results The hospitalization time (12.8±6.9) d] and the healing time of the fracture (16.4±3.8) weeks] in the study group were significantly shorter than those in the control group (17.7±8.3) d and (19.6±4.1) weeks]. The incidence of complications in the study group (4.3%) was significantly lower than that in the control group (19.1%). The excellent and good rate after treatment in the study group (97.9%) was significantly higher than that in the control group (83.0%). Conclusion Staged external fixation and plaster external fixation in treatment of open tibial and fibula fracture has a shorter healing time, which is beneficial for the early functional rehabilitation exercise and help reduce the complications. The treatment is a safe and effective, and is worthy of clinical use.
