摘    要:目的 研究0.12 MPa(表压)吸氧方案治疗压缩空气减压病的临床疗效.方法 患者分为0.12 MPa治疗组和对照组.其中0.12 MPa治疗组患者7例,盾构机压缩空气环境检修作业时发生减压病,使用0.12 MPa吸氧方案治疗;对照组患者10例,选自中国生物医学文献库1994至2008年7篇文献报道的病例,在压缩空气舱内暴露并执行规定减压程序后发生减压病,加压治疗压力为0.15~0.50MPa.结果 0.12 MPa治疗组7例均1次加压治疗治愈;对照组2例1次加压治疗治愈,8例加压治疗后复发并经多次加压治疗治愈.0.12 MPa治疗组初次加压治疗治愈率显著高于对照组(P<0.01).结论 对于执行规定减压程序发生的压缩空气减压病,早期使用0.12 MPa吸氧方案加压治疗可取得满意疗效,疗效优于传统的加压治疗方案.

Abstract:Objective To study the effect of hyperbaric oxygen treatment profile of 0.12 MPa (gauge pressure) on air decompression sickness (DCS). Methods The patients were divided into the 0.12MPa HBO treatment group and the control group. In the 0. 12 MPa HBO treatment group, 7 cases of DCS occurred in the tunnel workers during maintenance of tunnel borers in the hyperbaric air environment. Following onset of the illness, the 0.12 MPa HBO treatment profile was used to treat the cases. Ten cases of the control group were chosen from 7 articles in the Chinese Biomedical Database from 1994 to 2008. These cases of DCS occurred following exposure in the compressed air chamber and required decompression procedures. Then, they were treated at a pressure of 0. 15-0.50 MPa. Results All the 7 cases in the 0. 12 MPa HBO treatment group were cured with just 1 compression treatment. For those cases in the control group,2 cases recovered completely with just 1 compression treatment, while 8 cases had relapses following hyperbaric air compression treatment and finally also recovered with repeated compression treatment (including HBO therapy). Cure rate for first recompression in the 0.12 MPa treatment group was significantly higher than that of the control group (P <0.01). Conclusions For compressed air DCS cases that occurred despite compliance with required decompression procedures, early application of the 0. 12 MPa HBO treatment profile could achieve satisfactory results, and the therapeutic effect was obviously superior than that of the traditional recompression profile.
