空气和纯氧复苏方案在新生儿窒息中的应用价值比较

摘    要:目的:探讨空气和纯氧复苏方案在新生儿窒息中的应用价值。方法选取2009年1月至2012年12月在我院产科出生的85例窒息新生儿作为研究对象,随机分为空气复苏组和纯氧复苏组。空气复苏组首先采用空气复苏,如果空气复苏无效改为纯氧复苏;纯氧复苏组直接采用纯氧复苏。并记录所有新生儿出生后1 min、5 min、10 min时的Apgar评分,以及出生时新生儿脐动脉的血气、新生儿建立自主呼吸的时间、开始啼哭时间、出生后5 min时的心率。结果空气复苏组与纯氧复苏组的新生儿脐动脉血气的各项指标以及Apgar评分在1 min、5 min、10 min的差异均无统计学意义(P〉0.05);空气复苏组新生儿建立自主呼吸的时间和开始啼哭的时间明显少于纯氧复苏组,且差异具有统计学意义(P〈0.05),而新生儿5 min后的心率两组差异无统计学意义(P〉0.05)。结论空气复苏在临床治疗新生儿窒息中起到积极的作用,甚至优于纯氧的复苏效果。

Abstract:Objective To compare the value of air and pure oxygen resuscitation program in newborns with asphyxia. Methods Eighty-five newborns with asphyxia from January 2009 to December 2012 in our hospital were selected and randomized into the air resuscitation group and pure oxygen resuscitation group. The newborns in air re-suscitation group were first treated by air recovery, followed by pure oxygen resuscitation if the air recovery is invalid. The newborns in pure oxygen resuscitation group were directly treated by pure oxygen recovery. The Apgar score 1 min, 5 min, 10 min after birth, the umbilical artery blood gas analysis at birth, the time of neonatal establish spontaneous breathing, the time of start crying, heart rate 5 min after birth were recorded. Results The newborn umbilical arterial blood gas indicators, and the Apgar scores 1 min, 5 min, 10 min after birth showed no statistically significant differences between the two groups (P〉0.05). The time of newborns to establish spontaneous breathing and start crying in air resus-citation group was significantly lower than pure oxygen resuscitation group (P〈0.05). The newborn's heart rate 5 min af-ter birth showed no statistically significant difference between the two groups (P〉0.05). Conclusion Air resuscitation plays an active role in the clinical treatment of newborns with asphyxia, even better than pure oxygen resuscitation.
