非小细胞肺癌的化疗联合免疫治疗中树突状细胞与自然杀伤细胞间对话调控的机制研究

摘    要:目的探究非小细胞肺癌的化疗联合免疫治疗中树突状细胞(Dendritic Cells,DC)与自然杀伤细胞(natural killer cell,NK)间对话调控(Cross-talk)的调控机制。方法采用重复测量设计:非小细胞肺癌患者(性别不拘)需进行细胞治疗的患者60例。将60例病人随机分为两组(n=30):对照组,化疗前一周实施肿瘤DC疫苗联合CIK、NK细胞方案;试验组,化疗后一周实施肿瘤DC疫苗联合CIK、NK细胞方案。共设置4个测量时间点:治疗前24 h、治疗后48 h、治疗后1W、治疗后4W。使用ELISA法监测患者血清中可溶性细胞因子的表达,包括IL-12、IL-18、IL-15、分泌型TNF-α以及IFN-C。结果试验组和对照组血清中的IL-12、IL-18和IL-15在治疗前均无差异(P0.05)。实施治疗后,试验组患者血清中DC细胞相关可溶性细胞因子的表达较时间点的对照组数据明显增高,IL-12(P0.05),IL-18(P0.05),IL-15(P0.05);其表达在治疗后1W达到峰值,其峰值维持治疗后4 W(P0.05)。结论在化疗治疗之后进行免疫细胞治疗,免疫细胞间Cross-talk的调控诱发的DC细胞和NK细胞间的正反馈作用显著增强。

Study on regulation mechanism of cross-talking of NK and DC in non-small cell lung cancer by chemotherapy combined with immunotherapy
Abstract:Objective To study the regulation mechanism of cross-talking of NK and DC in non-small cell lung cancer by chemotherapy combined with immunotherapy. Methods The repeated measure design was adopted. 60 patients with non-small cell lung cancer were randomly divided into 2 groups (n=30). In the control group, the tumor DC vaccine combined with CIK and NK cells was implemented in the previous week. In the experimental group, the tumor DC vaccine combined with CIK and NK cells was carried out one week after chemotherapy. A total of 4 time points were set, including before and 24 h, 48 h and 4 w after treatment. ELISA method was used to moni-tor the expression of soluble cytokines in serum, including IL-12, IL-18, IL-15, TNF-αand IFN-C. Results There was no difference in IL-15, IL-18 and IL-12 between the experimental group and the control group (P<0. 05). After the treatment, the serum levels of DC cells, IL-12, IL-18, and IL-15 in the experimental group were significantly higher than that in the control group(P<0. 05), and the expression reached the peak 1w after treatment, and lasted for 4 Ws (P<0. 05). Conclusion After chemotherapy treatment, the positive feedback between DC cells and NK cells induced by Cross-talk is significantly enhanced by immune cell therapy.
