初治涂阳肺结核患者血小板相关指标研究

目的探讨血小板相关指标在初治涂阳肺结核(SPPTB)诊治中的意义。方法分别检测234例初治SPPTB患者治疗2月前后和39例健康对照组的血小板计数(PLT)、血小板平均体积(MPV)、血小板分布宽度(PDW)、大血小板比率(P-LCR)和血小板压积(PCT)5项血小板相关指标。分析指标的变化情况及其与痰涂片结果的关联性。结果初治SPPTB患者PLT、PCT较健康对照组均明显升高(均P=0.00),MPV、PDW、P-LCR较健康对照组均明显降低(均P=0.00);2月末痰涂片转阴组和2月末痰涂片未转阴组5项指标比较,差异均无统计学意义(均P0.05)。与治疗前比较,2月末痰涂片转阴组治疗后PLT和PCT均显著降低(均P=0.00),MPV、PDW和P-LCR差异均无统计学意义(均P0.05);2月末痰涂片末转阴组治疗后5项指标差异均无统计学意义(均P0.05)。初治SPPTB患者初次痰涂片抗酸杆菌阳性级别与PLT、MPV、PDW、P-LCR、PCT 5项指标均无相关性(均P0.05)。结论 PLT和PCT可作为初治SPPTB患者诊断和疗效评估的参考指标。

Study on platelet parameters in new smear positive pulmonary tuberculosis

Objective To study the significance of platelet parameters in diagnosis and treatment of new smear positive pulmonary TB(SPPTB). Methods Platelet parameters, including platelet count(PLT), mean platelet volume(MPV), platelet distribution width(PDW), platelet larger cell ratio(P-LCR)and platelet crit(PCT)were detected in 234 new SPPTB patients before and after two months' treatment and 39 healthy controls. The changes of five platelet parameters and its relationship with sputum smear results were analyzed. Results PLT and PCT in new SPPTB patients were significantly higher(all P=0. 00)than that in healthy controls. However, MPV, PDW and P-LCR in new SPPTB patients were significantly lower(all P=0. 00)than that in healthy controls, and showed no statistical significance between the negative smear conversion group and the persistent smear positive group after two months' treatment(all P>0. 05). PLT and PCT were significantly lower after the treatment(both P=0. 00), and MPV, PDW and P-LCR showed no statistical significance after two months' treatment in the negative smear conversion group(P>0. 05). All five platelet parameters had not statistical significance before and after two months' treatment in the persistent smear positive group(all P>0. 05). The results showed that there was no signif-icant relationship between acid-fast bacilli sputum smear-positive level and PLT, MPV, PDW, P-LCR and PCT in new SPPTB patients(all P>0. 05). Conclusion PLT and PCT in new SPPTB patients can be used as the reference parameters for the diagnosis and therapeutic effect.
