摘    要:目的重症肺炎患儿心电图检查QT离散度与患儿心功能指标变化的关系。方法选取本院96例确诊重症肺炎患儿,根据是否合并心衰,分为心衰组39例、肺炎组57例,同龄正常儿童50例作为健康组,分别测定三组儿童的心功能、心肌酶谱指标及心电图QT离散度并进行分析比较。结果心衰组患儿的QT离散度测定值显著大于肺炎组、健康组,且差异具有统计学意义(P0.05);心衰组患儿的CK-MB、ɑ-HBDH测定值显著的大于肺炎组、健康组,且差异具有统计学意义(P0.05);心衰组患儿心功能指标LVEF、FS、CI、PFVA测定值显著小于肺炎组、健康组,且差异具有统计学意义(P0.05);三组婴幼儿的PFVME测定值差异无统计学意义(P0.05);心衰组患儿心功能指标的QT值与LVEF、FS、CI、测定值均呈显著的负相关关系(P0.05);与PFVME、PFVA的相关系不明显(P0.05)。结论重症肺炎患儿心电图检查QT离散度测定,对于判断患儿心功能受损具有一定的价值,有利于患儿的诊治。

Relationship between QT dispersion and heart function in 96 cases of severe pneumonia
Abstract:Objective To explore the relationship between QT dispersion and heart function in patients with severe pneumonia.Methods 96 children with severe pneumonia were divided into the group A (39 cases of heart failure)and the group B (57 cases of pneumonia),and 50 healthy children at the same age were taken as the control group.Their heart function,myocardial enzymes indicators and QT dispersion were analyzed and compared.Results QT dispersion measurement values in the group A was significantly greater than in the group B and the control group (P <0.05).The value of CK-MB and ɑ-HBDH were significantly higher in the group A than in the group B and the control group (P <0.05).Their LVEF,FS,CI and PFVA were significantly lower in the group A than in the group B and the control group (P <0.05).There was no significant difference in PFVME among the three groups (P >0.05).QT value was negatively correlated with LVEF,FS and CI (P <0.05),but it showed no correlation with PFVME and PFVA (P >0.05).Conclusion QT dispersion measurement has a certain value for children with cardiac dysfunction judge,in favor of the diagnosis and treatment of children with severe pneumonia.
