γ干扰素诱导蛋白10对活动性肺外结核的辅助诊断价值

摘    要:目的探讨γ干扰素诱导蛋白10(IP-10)对活动性肺外结核诊断的临床价值。方法选取活动性肺外结核病例27例,活动肺结核病例81例,健康志愿者12例,予检测血IP-10水平,并同时检测各组血四项结核抗体(包括抗结核抗体、抗结核LAM抗体,抗结核38KD抗体,抗结核16KD抗体)、血T-SPOT水平,并进行统计分析。结果活动性肺外结核组的IP-10值为(248.42±167.13)ng/L,活动性肺结核组IP-10值为(200.74±150.71)ng/L,健康组IP-10值为(48.12±31.43)ng/L,活动性肺外结核组的IP-10值均高于其他两组,差异均有显著性(P均0.05),ROC曲线得出,曲线下面积(AUC)为0.960,IP-10诊断活动性肺外结核的临界值为54.68ng/L,其敏感性为96.3%,特异性为91.7%。结论γ干扰素诱导蛋白10,在活动性肺外结核中显著升高,可作为活动性肺外结核诊断辅助指标。

Clinical value of interferon-inducible protein-10 in the diagnosis of active extra-pulmonary tuberculosis
Abstract:Objective To investigate the value of interferon-inducible protein-10(IP-10)in the clinical di-agnosis of active extra-pulmonary tuberculosis. Methods The study selected 27 patients with active extra-pulmonary tuberculosis, 81 patients with active tuberculosis, and 12 healthy volunteers, and their level of IP-10 were detected and analyzed. 4 tuberculosis antibodies(anti-tuberculosis antibody, anti-tuberculosis LAM antibody, anti-tuberculo-sis 16kD antibody, anti-tuberculosis 38kD antibody)and T-SPOT levels were also investigated in active pulmonary tuberculosis. Results The serum IP-10 concentration in active extra-pulmonary tuberculosis was(248. 42 ± 167.13)ng/L,(200.74 ±150.71)ng/L in active tuberculosis,(48.12 ±31.43)ng/L in healthy controls(P<0. 05). In addition, the area under curve(AUC)was 0. 960, the critical value was 54. 68ng/L, and the sensitivity and specificity were 96. 3% and 91. 7% respectively. Conclusion Serum IP-10 increases significantly in active ex-tra-pulmonary tuberculosis, which could be used as an auxiliary index in the diagnosis of active extra-pulmonary tu-berculosis.
