呼吸道合胞体病毒抗体在哮喘患儿血清中的水平改变及临床意义研究

目的呼吸道合胞体病毒抗体在哮喘患儿血清中的水平改变及临床意义研究。方法连续性纳入自2013年1月至2014年12月于我院就诊并被确诊为哮喘患儿80例。另选取年龄,性别相对应的40例健康儿童作为对照组。检测哮喘患儿抗RSV-Ig G抗体和抗RSV-Ig E抗体水平改变。结果哮喘组患儿总Ig E(469.2±99.3 IU/m L vs.130.5±23.2 IU/m L,P0.05),抗RSV-Ig E(155.6±38.7 IU/m L vs.20.1±2.9IU/m L,P0.05)和抗RSV-Ig G(140.2±42.9 IU/m L vs.58.7±15.0 IU/m L,P0.05)水平明显高于对照组。ROC曲线分析提示抗RSV-Ig E的AUC为0.759,cut-off为177.5,敏感性为77.8%、特异性为79.2%。抗RSV-Ig G的AUC为0.723,cut-off为150.1,敏感性为81.0%,特异性为85.9%。相关性分析提示抗RSV-Ig E(r=0.393,P0.05)和抗RSV-Ig G(r=0.383,P0.05)与哮喘存在正相关。抗RSV-Ig E(r=0.442,P0.05)和抗RSV-Ig G(r=0.302,P0.05)与血清总Ig E水平存在正相关。抗RSV-Ig E和抗RSV-Ig G之间也存在正相关关系(r=0.421,P0.05)。Logistic回归提示抗RSV-Ig E(177.5 IU/m L)(OR=1.56,95%CI 0.99~2.29,P0.05)和抗RSV-Ig G(150 IU/m L)(OR=1.39,95%CI 0.95~2.53,P0.05)为哮喘发生的独立危险因素。结论 RSV隐性感染可以促发哮喘发作,其机制可能与抗RSV-Ig E表达增加有关。

Clinical significance of anti-respiratory syncytial virus antibodies in pediatric patients with asthma

Objective To evaluate the clinical significance of anti-respiratory syncytial virus antibodies in pediatric patients with asthma .Methods 80 pediatric patients with asthma were enrolled in this study , and another 40 healthy children were selected as the control group .The levels of IgE-anti-RSV and IgG-anti-RSV were measured . Results The levels of total IgE was 469.2 ±99.3 IU/mL in the study group and 130.5 ±23.2 IU/mL in the control group (P<0.05), the level of IgE-anti-RSV was 155.6 ±38.7 IU/mL in the study group and 20.1 ±2.9 IU/mL in the control group (P<0.05), and the level of IgG-anti-RSV was 140.2 ±42.9 IU/mL in the study group and 58.7 ±15.0 IU/mL in the control group (P<0.05).ROC analysis showed that the AUC, cut-off value of IgE-anti-RSV were 0.759 and 177.5 respectively, and AUC, cut-off value of IgG-anti-RSV were 0.723 and 150.1 respectively. Correlation analysis showed that both IgE-anti-RSV and IgG-anti-RSV were positively associated with total IgE .Logis-tic regression analysis showed that both IgE-anti-RSV and IgG-anti-RSV were independently risk factors for asthma . Conclusion IgE-anti-RSV responses may play important roles in RSV infection and asthma .
